PEM-like descriptions and accounts in non-ME illnesses

I thought It would be interesting to have a place to collect descriptions or accounts of living with other illnesses that include phenomena seemingly nearly identical to what we call PEM. For Mitochondrial disease, check out the thread: What differentiates ME/CFS from known primary...

    Is PEM related to exerkines?

Exercise leads to the release of a wide variety of substances, in the category of exerkines. https://www.nature.com/articles/s41574-022-00641-2 What if one or several of these exerkines were blunted in ME/CFS or, for some obscure reason, had a damaging effect instead of a health-promoting...

Comparing acute sickness symptoms with ME/CFS

I thought it might be worthwhile to have a discussion comparing and contrasting the symptoms of an acute infection (a week or two of flu, cold, staph, etc.) and ME/CFS. Viral persistence is often mentioned in discussions of mechanism theories. I personally think there's a good chance ME/CFS is...

Is PEM triggered by physical exertion the same as worsening triggered by sensory stimuli, cognitive exertion or emotions? What are the implications?

I have started this thread in order to reduce diversion of another thread where the subject arose. Here are a couple of quotes from that thread: My questions for discussion: pwME experiences: Is the worsening of symptoms triggered by sensory stimuli such as light and sound the same from a...

    Prevalence and measurement of post-exertional malaise in post-acute COVID-19 syndrome: A systematic review and meta-analysis, 2024, An+

Objective Post-exertional malaise (PEM) is a common and debilitating symptom of post-acute COVID-19 syndrome (PACS), and it is also included in the core outcome set for PACS. Increased hunger during PEM crash

I get much more hungry during crashes. Not sure if it's anxious eating, since my anxiety also increases, or related to some mechanism of ME. I have been averaging around 2000 calories for a few weeks. I'm in a crash now from some intense computer focus sessions the past few days, and I'm at...

Heresy / Conjecture on the nature of PEM PESE PENE

New here. An ME blogger suggested I bring this idea here for discussion. Will leave out 9/10ths of this idea, looking to see if there's interest in discussion, and most importantly...facts which make this wrong. There’s a protein called myoglobin. It is expressed on muscle cells. One end of...

Exhaustion in ME/CFS, what is it and what causes it - discussion thread

Discussion moved from this thread But the question is - what is 'exhaustion' in the context of ME/CFS. We have little evidence that it is actually depletion of energy reserves or a 'run down battery'. It certainly seems to feel like that but there is no shortage of oxygen or glucose and almost...

    Avoiding crashes as a treatment

    The “Permanent Worsening” Phenomenon

In the patient community, it seems somewhat common to refer to instances where PEM (or a crash) led to worsening of the condition which was permanent. I have however yet to find a reference to this in the literature, if someone knows of one, I would be grateful as it is of interest to me, thanks!
