First up, I didn’t spot any major issues apart from one missing sentence. Other than that I think you got the meaning across pretty well
So the comments below are mostly just musings about minor nuances – and it could well be that the problem is that my intuitions are starting to fail me. It’s more than a decade since I've been immersed in the French language and language transfer from English is a thing. For example, you translated ‘onset and course’ as ‘apparition et évolution’ and I had an immediate vision of some ghostly spectre of a ghastly apparition

In some instances the meaning is close to ‘proof’ and ‘preuve’ may be near enough but in other cases the meaning is closer to ‘basis’, ‘information’ or ‘indication’.
