I had a look through some of the questions and I’m not very impressed by the framing. Some of the questions seem to be framing recovery/improvment, as something the individual accomplishes by doing something instead of just random luck.

Questions that could be paraphrased to “If you are better, what did you do to get better”
 
I disagree, it asked if you returned to a full training programme? and if so how did you get there?
It also asked what advice you were given and did it help.

it is a survey of semi/professional athletes
 
So it’s the equivalent of asking, “if you returned to work, what did you do to improve to the point that was possible?” Which to me sends the wrong message, that improvement is something someone achieves by doing something.
