    There is an assumption in respect of medical records that they are accurate. Based on what I've read of my own medical records this is nonsense.

    If medical records for pwME are inaccurate, especially if they include negative statements that are incorrect, this amplifies the potential for stigma and negative attitudes from health care workers. It's crucial that everyone's records be accurate, especially so for the ME community where so many have been comprehensively PTSD'd.
