How do the various assays differentiate normal antibodies from autoantibodies?

Please explain it like I’m 15. Thanks. How much error is associated with these assays? Assuming that there are autoantibodies associated with ME/CFS why have they been so difficult to find relative to SLE, RA, Sjogrens, scleroderma, etc?

Daratumumab induces mechanisms of immune activation through CD38+ NK cell targeting, 2020, Viola et al

Abstract Daratumumab (Dara), a multiple myeloma (MM) therapy, is an antibody against the surface receptor CD38, which is expressed not only on plasma cells but also on NK cells and monocytes. Correlative data have highlighted the immune-modulatory role of Dara, despite the paradoxical...

Trial Report Plasma cell targeting with the anti-CD38 antibody daratumumab in ME/CFS -a clinical pilot study, 2025, Fluge et al

Not so much separation of the means. Looks like 225 versus 175 or so. Maybe no really low NK patients. But also larger sample size here—33 versus 10. But Fluge should have cited this in the Dara paper.
