Post-Exertional Malaise - a discussion including defining and measuring PEM

I can understand that. I don't have many symptoms so it's easier for me to identify a change in fatigue state and know when it's time not to go over my baseline. It took many years of trial and error figuring this out though

Post-Exertional Malaise - a discussion including defining and measuring PEM

I agree. Our experiences with PEM will differ. During the first several years of illness I would get a sore throat after over exerting, but that was during the PVFS phase. Symptoms change over time, and for those of us who have been ill for decades it's easily distinguishable.

Post-Exertional Malaise - a discussion including defining and measuring PEM

I was tested for many viruses via PCR/DNA/RNA but nothing ever showed up, even when I felt 'viral' or bad. The testing was done about one year after that EBV and HHV6 reactivated episode. It's difficult to test for I think unless you're very symptomatic? Dunno I hope they find something...

Post-Exertional Malaise - a discussion including defining and measuring PEM

I had very sudden symptoms one morning but not bad enough to stay home. I was feeling very well during that time period, about 90% improved in 2001. I had a doctors appt that day so went to see my doc who asked how I was doing and I said 'great', when I really wasn't sure why I feeling so...

Preprint System and Method to Determine ME/CFS and Long COVID Disease Severity Using a Wearable Sensor, 2024, Sun et al

These are the 2 most comfortable positions when I'm upright. I think it pumps blood back up? I can't sit with my legs dangling for very long, it's very uncomfortable.

Post-Exertional Malaise - a discussion including defining and measuring PEM

@duncan I'm not sure what you're asking. I was explaining to Eddie that over exertion PVFS in my case feels very different from delayed M.E PEM from going over my energy limits. If I have a viral infection over exertion I would experience immediate PEM but that does not include the same...

Faintly tired: a systematic review of fatigue in patients with orthostatic syncope, 2022, Ryan E. Y. Wu et al

Abstract Background Orthostatic syncope (transient loss of consciousness when standing—fainting) is common and negatively impacts quality of life. Many patients with syncope report experiencing fatigue, sometimes with “brain fog”, which may further impact their quality of life, but the incidence...

Post-Exertional Malaise - a discussion including defining and measuring PEM

Yes, but I couldn't stand for more than 2 minutes. I rested for 3 1/2 weeks and felt I had recovered and gone back to my baseline after one month so went out for a very nice 50 minute walk only to get up the next morning unable to stand up on my legs again. I had to crawl around and eat out...

Post-Exertional Malaise - a discussion including defining and measuring PEM

I contracted COVID in 2022 and it didn't resemble M.E PEM at all, not even close. No cognitive issues or other symptoms of delayed ME PEM. But, it was similar to PVFS over exertion I experienced during sudden viral onset in 1992. It affected legs muscles only, the only muscles I used for...

Viral RNA-Based Diagnostics: Nanopore System May Outperform PCR

The new diagnostic system combines Schmidt’s area of expertise in optofluidics, which is the control of tiny amounts of fluids with beams of light, with a nanopore for counting single nucleic acids to read genetic material. The tool was designed to test for Zika and SARS-CoV-2, which have been...

Preprint Plasma-based antigen persistence in the post-acute phase of SARS-CoV-2 infection, 2023, Peluso, Swank et al.

Although not without limitations, our data provide strong evidence that SARS-CoV-2, in some form or location, persists for up to 14 months following acute SARS-CoV-2 infection. This persistence is influenced by the events of acute infection. These findings motivate an urgent research agenda...

Yes :emoji_hand_splayed: and these are the pwME who eventually reach the point of no return when they might have had a small chance of returning to work part-time or having a small social life early in the illness if they paced properly and didn't exercise. I continued to run for 2 yrs, but...
