Agomelatine but not melatonin improves fatigue perception: A longitudinal proof-of-concept study, Pardini et al, 2014

Didn't we see somewhere that CLOCK expression is dysregulated in pwME? Does anybody know more about that and if the mode of action of agomelatine mechanistically could have a positive influence on us in that regard?

Agomelatine but not melatonin improves fatigue perception: A longitudinal proof-of-concept study, Pardini et al, 2014

That would definitely be something to look into further. As a starting point it sounds pretty good that they didn't observe adverse effects. I could imagine if 30 ME/CFS patients take Adderall for three months, there would be adverse effects?

Agomelatine but not melatonin improves fatigue perception: A longitudinal proof-of-concept study, Pardini et al, 2014

Unfortunately, it's not available in the US, which surely hinders adoption/testing. I will ask my doc whether it could make sense to test it.

Amisulpride vs. fluoxetine treatment of chronic fatigue syndrome: a pilot study, Pardini et al 2011

Low dose amisulpride (as used here) could work similarly to low dose aripiprazole if dopamine receptors play a role?

Agomelatine but not melatonin improves fatigue perception: A longitudinal proof-of-concept study, Pardini et al, 2014

Fulltext translated from Russian with DeepL. Prospective observational trial from 2024 of post-COVID depressed patients. Only adding it, as it was mentioned in the talk.

Agomelatine but not melatonin improves fatigue perception: A longitudinal proof-of-concept study, Pardini et al, 2014

From what I've been able to gather on agomelatine, it's a bit debated whether it really shows any 5-HT2C antagonism at clinical doses. The ki for the MT receptors are subnanomolar, where the ki for 5-HT2C is an order of magnitude higher. But apparently MT-2 and 5-HT2C form heterodimers, so...

Agomelatine but not melatonin improves fatigue perception: A longitudinal proof-of-concept study, Pardini et al, 2014

They briefly mentioned this paper and a Russian one in the ME/CFS conference in Berlin, I just found the video. From around minute 6:15! This is the other paper he mentions in his presentation. Observational study on depressive episodes after COVID - includes a 10 point improvement in...

Just saw this on Reddit and realized it apparently hasn't been shared here.

Agomelatine but not melatonin improves fatigue perception: A longitudinal proof-of-concept study, Pardini et al, 2014

Chronic Fatigue Syndrome (CFS) represents a disabling condition characterized by persistent mental and physical fatigue, bodily discomfort and cognitive difficulties. To date the neural bases of CFS are poorly understood; however, mono-aminergic abnormalities, sleep–wake cycle changes and...

    Mestinon

Just a boring update: It's still working very well. Standing around 80 BPM now (from 120-140 bpm before treatment) - quality of life greatly improved.

Open Norway: Plasma cell aimed treatment with daratumumab in ME/CFS (ResetME) - Haukeland University Hospital

From what they told me, you must be in the Norwegian Health Care system, hence the requirement to be a resident. I'm not sure how quickly one could enter that. They did not tell me any language requirements, I don't think you have to be/speak Norwegian from that they told me.

[Transcriptome] in PBMCs of [LC] at a median follow-up of 28 months [...] reveals upregulation of JAK/STAT [...], 2025, Fineschi et al

Jak/Stat is a (the?) downstream signalling pathway of gamma IFN - I guess that would tie in with Jonathan's hypothesis.

    Teclistamab for ME/CFS

Yeah, no idea what that is supposed to mean! Yes, the "T-cell engager" part I mention in my first post is referring to the CD3 part :) I can't speak for anything else you said regarding safety, potency or tissue penetration. I haven't look into it.
