Multiple system atrophy (MSA) is the most aggressive neurodegenerative disease of alpha-synuclein deposition. Autonomic failure is a cardinal feature of MSA and manifests as neurogenic orthostatic hypotension (nOH) and/or urogenital dysregulation. nOH is debilitating in MSA, leading to reduced quality of life due to orthostatic intolerance, syncope, falls, and a vicious cycle of deconditioning. Considering that MSA is primarily characterized by central autonomic failure, treatments focusing on pre-ganglionic targets should yield the greatest efficacy.
