I've only skimmed this article by Richard Smith, former BMJ editor, but thought it looked v interesting, is quite long, so prob not poss for everyone to read, energy-wise. (I hope I am not duplicating - I actually had to go to HOW TO help menu as I so rarely start threads and didn't know what to do!)
