My son repeated this little biochemistry department maxim a number of times when he did his biochemistry course years ago: medicine is what you do when you can't manage biochemistry.
An alternative diagnosis is on my mind a lot. Even 35 years after a diagnosis of 'CFS/FM' I won't be able to accept it until there's a definitive biomarker.
