Brain Retraining treatment for ME/CFS and Long COVID - discussion thread

Yes, and I have always done that here, as you can see from my posts. I don't reference every single thing I say, as you don't either. If there is anything specific that you would like a source for, please ask for it. If I miss your reply, send me a PM.

Brain Retraining treatment for ME/CFS and Long COVID - discussion thread

I think you have a number of misconceptions here. These programmes aren't saying random things about the brain. They are based on proven neuroscience, e.g. that fear, stress and expectation upregulate pain. Also, it's not about gaslighting yourself, ignoring symptoms or not trusting your...

Because patients are saying it all the time. I'd prefer not to point out individuals, but if you look you will see it is true, and I can certainly point out specific examples in private. (One was mentioned earlier, but was removed). There are many studies that have looked at this as well, and...

Effect of Pain Reprocessing Therapy vs Placebo and Usual Care for Patients With Chronic Back Pain an RCT, 2021, Asher, Gordon et al

Actually, this is not true (and I've given detailed and logical reasons why) and is not the opinion of the wider scientific community. It's a little odd that it is the overwhelming opinion of s4me, which is why I only spend a limited amount of time responding here. Perhaps you should invite more...

Effect of Pain Reprocessing Therapy vs Placebo and Usual Care for Patients With Chronic Back Pain an RCT, 2021, Asher, Gordon et al

No, it's not true that they can't provide "any useful information". As I said, with appropriate methodology, bias can be minimized, and you can provide useful information.

Effect of Pain Reprocessing Therapy vs Placebo and Usual Care for Patients With Chronic Back Pain an RCT, 2021, Asher, Gordon et al

The article gives references to the current state of pain neuroscience in the first four paragraphs. How exactly is the study not consistent with that evidence?

Effect of Pain Reprocessing Therapy vs Placebo and Usual Care for Patients With Chronic Back Pain an RCT, 2021, Asher, Gordon et al

Yes, I agree the placebo is terrible in this case. I agree with another poster that it should be a different psychological treatment, perhaps relaxation therapy.

Effect of Pain Reprocessing Therapy vs Placebo and Usual Care for Patients With Chronic Back Pain an RCT, 2021, Asher, Gordon et al

Sorry, there are a lot of replies and I don't really have time or motivation to go through them all. I'm aware of the issues. The point is that they can be mitigated to a certain extent (e.g. having an attention control as a comparitor, as in this trial). There will always be bias when there are...

Brain Retraining treatment for ME/CFS and Long COVID - discussion thread

No, I've never said any of that. It just sounds like a strawman. Please don't assume I'm not intimitely familiar with the illness (I am). This is about acknowledging that certain factors may be important for some patients and they should be addressed. It does NOT mean we should automatically...

Brain Retraining treatment for ME/CFS and Long COVID - discussion thread

That's great, but it is an issue for many patients. It's a mistake to think that everyone's illness is exactly the same, or that "worry about the illness" is the only problem (it isn't), or that it doesn't affect a significant number of patients (it does).

Brain Retraining treatment for ME/CFS and Long COVID - discussion thread

I'm well aware of that. The point is that stress doesn't cause one specific symptom (or syndrome), and is unpredictable.

Brain Retraining treatment for ME/CFS and Long COVID - discussion thread

But ME/CFS isn't typically like the flu (although can sometimes have flu-like symptoms). Lying in bed can definitely cause pain, fatigue and POTS. Worry about your body being broken can definitely cause symptoms. Those can be addressed. It's naive to think that your body will magically recover...

Effect of Pain Reprocessing Therapy vs Placebo and Usual Care for Patients With Chronic Back Pain an RCT, 2021, Asher, Gordon et al

The main point you make is that it is subjective, and yes it is, but that is the nature of pain. There are no objective measures. We have to believe the patients when they tell us they are in pain, and when they are in less pain. Yes, that is subject to bias. That doesn't mean the study is...

Why aren't they all suffering from PTSD, sleep, disturbance, migraines, chronic fatigue, depression, anxiety, chronic pain, tension headaches, or IBS, all of which can be caused by stress? Oh wait, they are! Around 75% of Americans reported to the American Psychological Association that they...

Effect of Pain Reprocessing Therapy vs Placebo and Usual Care for Patients With Chronic Back Pain an RCT, 2021, Asher, Gordon et al

Check one of my replies to you earlier today earlier in this thread. I never claimed pwME have an "emotional attachment to pain". I was simply explaining that pain in general has an emotional component. This isn't controversial or in doubt.

Yes, I read the first page of critiques, and didn't find them very convincing. Did you? Calling the authors idiots and the treatment brainwashing. Pointing out changes in alcohol consumption, even those weren't statistically significant.

Sorry if I misremembered your comment, and I can't find it now. Stress is not simply about trauma, it's about combined load. Exercise, infections, work, lack of sleep, etc. all add to that load.
